4. The date on which balance is loaded or transactions was made.

5. The amount of tax inclusions.

6. The total amount spent or used in the whole month along with the reimbursement amount.

7. Any additional purchases made on mobile phone.

8. Receipts should be attached behind the report or along with the document (optional).

Mobile expense sheet templates are handed over to the manager of the organization as a document either through E-mail or by hand and then printed out for use manually. The more to the point the report is, the easier it is for the management to reimburse their employees.
